MISSION AND GOALS
The New Hampshire Falls Risk Reduction Task Force is a coalition of professionals from across disciplines, all sharing a commitment to reducing the risk and numbers of falls among the state’s older adult (65 plus) population.
Our goals are to:
- Reduce the rate of death and disability in the older adult due to falls.
- Reduce the risk of falling in the older adult population.
- Educate and train professionals working with the older adult on falls.
Over the years, we have been involved with many projects, including, but not limited to, an annual professional conference, trainings on best practice for acute, long-term care, and community based settings, an evidence based team project, development of a curricula for older adults on falls risk reduction and a health communication campaign “You CAN Reduce Your Risk of Falls”. We are part of the national Falls Free Coalition facilitated by the National Council on Aging and our members have presented on falls statewide, nationally, and internationally.
All projects are evaluated, which include process and outcome evaluations. These can include number of falls and scores on screenings such as the Timed Up and Go. Some of the more global measures include:
- Deaths, hospitalizations, and emergency department visits due to falls.
- 911 calls due to falls.
- EMS runs due to falls.
The Task Force meets the first Tuesday of every month for two hours at 9 am at 29 Hazen Drive in Concord, New Hampshire. All are welcome.
